To reserve your space for the ETSU Soccer Academy, please fill out all information on the following form. If mailing in a check, please make sure to include the campers name on the bottom left line. There is a late registration fee of $15 for all Day Camp applications received after June 1st. There is a late registration fee of $25 for all Elite Camp applications received after July 1st. If this is not paid at time of payment, you will be charged at registration. Remember that you also need to mail in the Parental Consent Form to complete registration. If paying by PayPal, the full amount will be charged.
